     Do not enclose a freight payment if you are not ordering
     printed documentation.

     YOU MUST ENCLOSE A FREIGHT PAYMENT for products that
     contain printed documentation, including MAXDOC, MAX2D,
     MAX4D, MAX8D, MAX16D and MAXUD.  For each printed manual,
     you must enclose the freight fee given below:

PAYMENT BY CREDIT CARD:
  Overseas users may pay by VISA or American Express.  Your card
  will be billed in Canadian dollars, so you must use the Canadian Dollar
  order form.

PAYMENT BY WIRE TRANSFER:
  If you wish to pay by wire transfer, you must still submit a copy
  of this order form and indicate the date and amount of the
  transfer.  Please contact Lanius Corporation for bank account
  information.

PAYMENT TERMS:
  Make checks payable to Lanius Corporation.  Personal and company
  checks require two weeks to clear.  Corporate purchase orders are
  accepted but they must be accompanied by this order form.

FOREIGN EXCHANGE NOTE:
    To pay in a currency other than U.S. Dollars, Canadian Dollars,
    or Deutsche Marks, you can calculate your own local
    currency equivalent of the U.S. price.  Then submit a check or
    bank draft for that amount in your local currency.

    YOU WILL NOT BE CHARGED for foreign currency payments as long as:

    1) Your check must be COMPUTER-ENCODED at the bottom.

    2) The currency of your check MUST BE THE SAME as the currency
       of the COUNTRY where your check was drawn.

    3) YOUR CHECK MUST BE WRITTEN IN ONE OF THE FOLLOWING
       CURRENCIES:  Canadian Dollars, U.S. Dollars, Hong Kong
       Dollars, Deutsche Marks (including Austria), French
       Francs, Swiss Francs, U.K. Pounds Sterling, or Japanese Yen.

    IF YOUR CHECK DOES NOT MEET ALL OF THESE CRITERIA, you must
    enclose an extra US$40 fee (or local currency equivalent) with
    your registration.
